WebIf it's on the tongue, it's not likely to be a fever blister (AKA cold sore). More likely it is a canker sore, which unlike the fever blister isn't caused by herpes and is not contagious. Personally I tend to get painful enlarged papillae, but I don't think those are what OP is describing as I don't remember seeing a white head on them.
